America On Stage Liability Release

I, the person registering a dancer with America On Stage, or the person registering myself with America On Stage as a dancer, a director of a studio or otherwise responsible adult for participants in America On Stage events, do hereby give permission for my students, dancers, child, children, and all participants I am responsible for to participate with America On Stage in the dance competitions and events with which I am registered and/or participating.

            He/She/They is/are in good physical health and has no serious medical problems.

             Should it be necessary to receive medical treatment for my child, I hereby authorize the leaders of America On Stage, the facility managers or otherwise responsible adults to obtain such, or to contact available directors, parents or otherwise responsible adults to obtain such.

             I authorize America On Stage to provide such supervision as it deems reasonable and necessary for my students, dancers, child, children, and all participants I am responsible for safe participation in this event.

             I authorize America On Stage and all of its directors, leaders, and helpers, whether paid or volunteer from any claim, liability, or suit arising out of this event. I further agree to be responsible for the actions of my students, dancers, child, children, and all participants I am responsible for while participating in this event, and will hold America On Stage, StarWorks Productions Inc. and all of its directors, leaders, and helpers, whether paid or volunteer, fully and completely harmless from any claim, liability, or suit arising from the actions of my child.

 By registering with America On Stage, I agree to this liability release and all terms associated with it.
